The key word is facilitate, if that is all a peer or case manager is doing does it really matter who facilitates. Many consumers can write an advanced directive on a form without a facilitator, facilitators don't make decisions they just guide through the process. Answering questions and clarifying what the document is asking for...
Your case manager or therapist your psychiatrist your advocate (person with durable power of attorney) all need to know of the exsistance of the document and where it is to use the advocate for hte decisions the consumer set out
Would you sign a document you didn't endorse and if you did were you competent to do so if you didn't beleivein the desires requested?
