What kind of form is needed to write a PAD?  Some states recommend forms and others don’t.   The Bazelon Center includes a sample form, can that be used in states that don’t require certain forms or notarization procedures?  Can someone write a PAD on any sheet of paper?  How can we find out?  What does your state allow?

I just followed the headings in the state laws just to be sure what I did was legal. The Bazelone Center has a form you can use which is good also.

First, check your state which may require use of their own forms as well as notarization and witnesses. Second, contact your state's P&A to see if they have devised any forms for you to use for a PAD. Third, use the comprehensive form on the Bazelon Center website to guide your writing a PAD. These three steps should help answer this question.
