There is no official form for appointing an agent, but you must create a written document.  A helpful example of a form, which you may wish to use or adapt, is available here .

Although Colorado does not currently have a specific statute for a psychiatric advance directive, you can learn more about general advance directives for health care in Colorado at the Caring Connections website.
